Each August, the chapters are required to submit chapter reports to the State Association for submission for the 990 taxes. The NYSSFA Chapter Annual Report outlines the information needed by the State Association. This includes

  • Chapter Leadership – contains fields for names and schools of each officer and the location of Chapter meetings
  • Financial Information – a separate Chapter Financial Statement contains fields for checking and saving account figures as well as a breakdown of the income receive and expenses paid during the fiscal year (July – June). There is also a step-by-step list of instructions for filling out this form.
  • Chapter Membership – requests the latest list of the Active, Affiliate and Associate chapter members’ contact information
  • Workshop & Meeting Dates – requests the dates, times and locations
  • Chapter Scholarship Program – requests the number of scholarships given out along with the scholarship amount
